CONCERT UNIFORM ORDER FORM (LADIES) – Click to print, complete, and submit it with a check payment. Dress order is typically placed at the beginning of the performance season at the orientation. A uniform order form and a fee for the dress are required to place an order.

The ordered dress will need alterations for a better fit, especially in the length, as it will arrive unhemmed or long.

YSDP students must wear the appropriate formal concert uniform for the fall, winter, and spring concerts and other performances throughout the year, such as Chamber Ensemble Performances. The uniform is the student’s property and can be worn in future years.

YSDP has limited quantities and sizes on hand for sale to remedy emergencies for female uniforms. Please email info@ysdp.org if you have questions.
